These Terms of Service govern your use of this website, operated by Smile4Me Dental Care. By using the site and submitting the qualification survey, you agree to these terms.

The promotion

The Straight Smile promotion offers Invisalign from $4,500, which is $1,500 off the regular price of $6,000. The promotion includes a free consultation, a free 3D digital scan and a free customized treatment plan. More complex cases can cost more. Your exact price and timeline are given to you in writing at your free consultation before you decide anything. This price is open for the next two weeks.

Your information

When you submit the survey, you provide your name, email, phone number and survey answers. You agree to be contacted by call or text about your spot. There is no obligation to start treatment. We never sell your information.

No guarantee of qualification

Completing the survey does not guarantee you qualify for the promotion. Qualification depends on whether you can visit our Astoria office and whether you are ready to start. The final decision about your treatment and price is made at your free consultation.
